To cancel the contractual relationship, we will need a copy of the death certificate. You will receive this from the responsible doctor, hospital or nursing home.

Upload a maximum of 4 files (no bigger than 8 MB in total). The formats JPG, PDF, PNG, GIF and BMP are supported.

Your data will be kept confidential and will only be used to process your request.
